About the program:

Community Interlink is a partnership between local hospitals and health services across central and northern Victoria and southern New South Wales. Together, we’re committed to making sure people in our region have access to locally-governed Home Care supports—right where they live.

One of the ways we do this is through our Service Support Coordinators.

These team members play a key role in helping older people manage their Support at Home funding. They work closely with Case Managers to connect clients with the services they need—whether it’s arranging help around the house, organising transport, or linking people to social activities.

 

Our Support Coordinators:

  • Help clients stay independent and safe at home
  • Organise services through Purchase Orders
  • Check in to make sure people are happy with their care
  • Provide friendly, ongoing support

It’s all part of our mission to keep people well-supported, connected, and living comfortably in their own homes for as long as possible.
