About the Role:

An opportunity exists for a motivated Outreach Clinical  Pharmacist to join the Ambulatory Aged Care Programs (AACP) .

This position is to provide high-quality pharmaceutical care by optimising pharmacotherapy to achieve a safe patient outcome to the client’s managed in their home under the Ambulatory Aged Care Programs (AACP). You will be professionally reported to the Director of the pharmacy and operationally to Manager AACP. The position will be working with the multidisciplinary team to conduct a home visit by hospital pharmacist to comprehensively assess all aspects of a client’s medication management, support team in improving clients compliance and also provide education to patient and their carer.

 

The successful applicant will have:

  • Ability to work in a multidisciplinary team to support different AACP programs.
  • Current drivers licence.
  • Registered as a practising pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA) and a minimum of two years’ experience in clinical pharmacy service.
  • Demonstrated effective interpersonal skills, including communication, negotiation, team building, influencing, conflict resolution and the building of positive workplace culture.
  • Demonstrated skills and experience in managing a complex workload, quality improvement activity, problem solving and development and implementation of new initiatives.
