About the role:
An exciting opportunity exists to work within the team at the Shepparton PARC - (Prevention & Recovery Care)
The suitable applicant with need to demonstrate strong clinical & assessment skills, ability to work collaboratively with Wellways partners.
A PARC Clinician provides a critical role in the active coordination of care for people requiring a PARC admission for Step Up” from the community or “Step Down” from the inpatient unit and will be required to communicate with referrers & treating teams as required to ensure that service continuity is maintained for participants of PARC, staff and family members.
