Community Interlink is a partnership between regional hospitals and health services from central and northern Victoria and southern New South Wales.
The alliance assures the availability of locally-governed Home Care Package services and NDIS support coordination is available to communities within our region.
Community Interlink has an opportunity for an NDIS Support Coordinator or experienced Aged Care Case Manager to Join the team at Seymour, Benalla, Shepparton or Mansfield. Community Interlink is a supportive team of skilled individuals who will nurture you and help you to upskill and build your future in the transforming NDIS and Aged Care sector. This role provides guidance and support to our vulnerable community members to help them to identify supports that promote independence and attainment of goals.
This role is flexible and post completion of training may have the opportunity to work from one of the other Community Interlink outlets servicing the Southern catchment of our region.

About the role:
The Community Interlink Team support local people to access services through their funding which is received from both State and Commonwealth Governments. These services are available via the Department of Health and Aged Care and the National Disability Insurance Agency (NDIA) funded support programs. The services are specifically to maintain safety, choice, dignity and independence in their home for as long as possible. In this role you will have a weighted list of consumers and you will be responsible to ensure they have all the information required to select the best care options for them and provide ongoing support to ensure their needs are met. You will utilise your knowledge to make assessments and recommendations on behalf of your consumers. You will need to have exceptional communication and record keeping skills.
Successful candidate will have:
- A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience.
- Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
- Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
- A sensitive approach to working with individuals and a philosophy of empowerment.
- Knowledge of the Case Management approach to service delivery, in a rural community setting.
- Ability to work as part of a multi-disciplinary team.
