About the role
Community Interlink is a partnership between regional hospitals and health services from central and northern Victoria and southern New South Wales.
The alliance assures the availability of locally-governed Support At Home and NDIS supports to communities within our region.
The Service Support Coordinator role is an administrative role that supports Aged Care clients to manage their Support At Home funds through coordination of supports.
As a Service Support Coordinator you will work with Care Managers and Clients to maintain their independence by connecting them to services and monitoring their satisfaction. In this role you will be part of a vibrant hard working team working to ensure our consumers are well supported to maintain independence in their homes.
